- Holotype: QCAZ 120, adult ♂, skin and skull, collected by Felipe Campos on 26 June 1990.
- Type locality: “Ecuador, Provincia Santo Domingo de los Tsáchilas [in error, Provincia Esmeraldas], Bosque Protector La Perla at 200 meters of altitude (0º08'N, 79º30'W), 2 km south of La Concordia”.
