- Holotype: USNM 370119, adult ♀, skin and skull, collected by A.L. Tuttle and M.D. Tuttle on 18 August 1965.
- Type locality: “Pico Ávila, 2150 m, 5 km NNE of Caracas (= 'Hotel Humbolt [sic], 9.4 km N Caracas'), Distrito Federal, Venezuela”.
