Type locality: USA, Washington State, Lewis County, Gifford Pinchot National Forest, Forest Service road 25, 4.6 miles south of Forest Service road 300, Tributary of Iron Creek.

Holotype: AMNH male.

Paratypes:

Distribution: USA, Washington State, Southern Cascade Mountains in the Cowlitz River (includes Iron Creek) and Coweeman River (includes Goble Creek) of Lewis, Cowlitz, and Skamania Counties.
