- Holotype: THNHM 15905 [formerly MS 390], adult ♂, collected by Kirati Kunya on 5 October 2009.
- Type locality: “Ban Bangrong, Thalang District, Phuket Island, Phuket Province, southwestern Thailand”.
- Etymology: phuketensis: named after Phuket island (Phuket province), Thailand, where this species was discovered.
