- Holotype: KU 164702, adult ♂, collected by by William E. Duellman, Alan H. Savitzky, and John E. Simmons on 7 April 1975.
- Type locality: “3.5 km (by road) northeast of Mindo, 1540 m, Provincia de Pichincha, Ecuador (00°01S,78°44′W)”.
