- Holotype: USNM 461459 (field no. T-391, L5049), adult ♀, skin and skull, collected by personnel of the Institute Evandro Chagas on 8 May 1969.
- Type locality: “Rio Amapari, Serra do Navio (0°59' N, 52°03' W), Amapá, Brazil”.
