Type locality: "lower montane rain forest at 1700 meters elevation, south-west of the Río Azuela bridge on the Quito--Lago Agrio road, eastern base of Volcán Reventador, Napo Province, Ecuador (latitude 0° 05´ S, longitude 77° 37´ W)".
Holotype: AMNH 89603.
