- Holotype: NEIU MCP 126, whole specimen.
- Type locality and horizon: Pennsylvanian age, from the Francis Creek Shale, Carbondale Formation of Will County, Illinois, USA.
Etymology: Named probably after D. Siroka and/or S. Siroka (author thanked both in the description for loan of the specimen), if named after both, specific epithet should be changed to sirokaorum.
