• Holotype: MUSM 23757, adult ♀, skin and skull, collected by Carlos Tello on 15 April 2006.
• Type locality: “Campamento Alto Parramata at 2630 m elevation, in Carmen de la frontera district, Huancabamba province, Piura department, Peru, 4.89429° S, 79.36761° W”.
